I agree, don't get worked up about it yet. Wait and see what the surgeon says, and you'll find out whether or not you can be flexible about when the surgery takes place and how long recovery will take. Also think about what you can possibly do in a work from home scenario. If you think your boss is a good person, then talk to them after you know more.

It would suck to lose this job, but your back pain has gotten worse and less manageable over time and you can't do your job well if you're struggling to get through your day. Prioritize your health first.
