Honestly? Not everybody knows how to deal properly with such situations. It's called being clueless. It does not make them an asshole. That would be if they used the wrong pronouns despite better knowledge.

Yes, being clueless does not excuse bad behaviour, and it is hurtful (and sometimes even more hurtful than intentional acts, because cluelessness can often be among people one actually knows and trusts), but there is a difference between doing it because they are used to calling him "she" or with an intent to hurt them.

Add to that that the OP knew him for a couple as years as a woman, and you can mostly blame this on a mix of habit and retrospective reference.
